Veteran Banners cost:

$63.00 (Credit/Debit)

or $60.00 (Cash/Check during in person event)

Eligibility

The individual must have served in any of the following branches of the US Armed Forces (Army, Marine Corp., Navy, Air Force, Coast Guard, Space Force, National Guard, or Reserves) and must also meet the following conditions:

*Current resident of Gallia County OR resident at time of entry.

*Be an honorably discharged veteran OR currently serving veteran.

Application Guidelines

The Hometown Veteran’s Banners are funded by individual payments. Donor contributions help with the cost for banner mounts (to expand towards possibly 800 or so lamp pole / utility pole mounts), consumables, maintenance and administration, web site hosting, rental unit storage for safe, secure, dry storage of banners when not on display, and related expenses.

The cost of each banner is $63 (credit/debit) or $60 (cash/check during in person event).

To apply please either:

(1) go to the online application and order form and upload your Veteran Photo and redacted (Black-out SSN and address) copy of DD-214 or alternative below.

or

(2) download the Banner Application.pdf and the Banner Release Form.pdf, complete the forms print with ink pen and sign, and email or bring to an in person event:

• High-resolution picture preferably an 8X10 of your veteran in uniform if possible.
• Redacted (Black-out SS# and address) copy of the Veteran’s proof of military service, DD 214 (post 1950), WD AGO Form 53-55 (prior to 1950), National Archives Records Administration NA Form 1338.
• Current Active Duty/Reserve/National Guard please provide USA: Soldier Record Brief (Enlisted (ERB)/Officer Record Brief (ORB)), USN: Performance Summary Record (PSR) or other proof of medals, wars, operations.
